Forms

Forms are used when input is required from a user. Examples include surveys, feedback forms and application forms.

Tell Me More?

Forms can be provided in a number of ways. These include formats that can be:

printed, filled in by hand and then faxed, delivered by hand or posted to the agency

filled in online, printed and then faxed, delivered by hand or posted to the agency

filled in online and submitted electronically.

Forms may also automatically run simple checks on the information supplied, to ensure that it is complete and correct, and prompt users.

Cost, the complexity of the form, the nature and size of the user group, authentication and security may influence decisions about the most appropriate online formats for particular forms. Any one form may also be presented in multiple formats.

Agencies may also consider the potential cost savings that online forms can provide when the need for agency staff to key in data and to follow up incomplete or incorrect forms is reduced.

Why Must I?

The Online Content Requirements require that Australian Government departments and agencies provide various types of information online, including all forms for public use. Compliance with the Online Content Requirements guarantees the public access to a minimum level of information across all Australian Government agencies.

There is a limited range of circumstances where it may be inappropriate for a form to be completed online or provided as a download. If it is not possible to provide online access then information on alternative ways of obtaining and completing the form must be provided.
